Director of Client Services:
Northern Colorado AIDS Project (NCAP) is seeking applications for the full-time position of Client Services Director in the case management department. This position provides direct case management to HIV positive individuals, supervision of the case management team, grant management and reporting, and representation on state committees with other AIDS Service Organizations. This position will operate as an integral part of the NCAP team and will participate in NCAP staff meetings and other administrative/special events held by the agency. Travel required within the northeast 8-county region of Colorado.

Starting salary: $32,000-34,500/year commensurate with experience and qualifications.
Requirements: Master’s of Social Work along with relevant work experience preferred. Individuals with equivalent experience will be considered. Overview of Case Management Services:
NCAP provides medical case management services in the 8 counties of northern Colorado. This position will be responsible for all aspects of client case management as defined by the Ryan White CARE Act. Case management services are provided in a team environment to ensure quality and culturally competent services are provided according to the agency’s mission and within funding guidelines. Staff is expected to follow the standards of care prescribed in the Ryan White Title II Care Act, keep accurate records in a timely manner, and participate in clinical and case management supervision.
SPECIFIC JOB RESPONSIBILITIES
Specific Responsibilities (Case Management):
* Conduct intakes and perform psychosocial and needs assessment and establish eligibility for grant funded financial assistance programs.
* Create wellness plans with clients to reach short-term and long-term goals.
* Complete ongoing data entry in HELIX documenting client centered activities.
* Connect clients with medical, mental health and substance abuse treatment as needed.
* Make appropriate referrals for housing assistance.
* Maintain case files in an accurate and timely manner.
* Participate regularly in clinical and case management supervision.
* Attend trainings as available.
* Maintain confidentiality of all clients.
* Ensure safety of clients, volunteers, and employees of NCAP.
* Regional travel required.
* Other duties as assigned.
Supervisory, Management and Administration Duties:
* Provide assistance and support to other case managers in performing their jobs.
* Ensure appropriate standards of performance are maintained by case managers. Assist in establishing goals and objectives for department and case managers. Participate in evaluation of department meetings to discuss current client issues, training needs, and review/approve client requests.
* Participate in preparation of annual Ryan White Care Act grant with Executive Director. Participate in other case management related grant writing as needed.
* Manage case management grants and monitor annual spend down sheets.
* Monitor HELIX database and prepare reports as needed.
* Promote client advisory board meetings and provide assistance to accomplish goals.
* Maintain relationships with local universities and supervise interns and assist them in completing their programs.
* Attend statewide and regional meetings to represents the organization
Qualifications:
* Master’s degree in Social Work or related field preferred.
* Experience in the area of human services, social work, counseling, or related field.
* Commitment to the principles of harm reduction and providing confidential, non-judgmental services.
* Basic knowledge of computers and computer software.
* Open to engaging in clinical supervision in order to better perform job duties.
* Desire to be a member of a team environment.
* Ability to work flexible hours including evenings and occasional weekends.
