Case Management and Social Services
We believe that living an active, meaningful life as an HIV-positive person is not only completely possible, but that we can help in that process by providing resources, support, and assistance.
Our case managers know the many challenges of the medical and social systems, and can provide information, counsel, and advocacy. NCAP keeps all client information completely confidential and works to ensure that every client can feel safe, comfortable, and at home when visiting our office.
Who can be a client of NCAP?
NCAP case managers are available to serve all men, women, and children living with HIV/AIDS in the eight-county region of northern Colorado. The agency also serves partners and families of HIV-positive clients. Bilingual services are available for our Latino clients (se habla espanol).
NCAP has two offices to better meet everyone's needs. Our main office is in Fort Collins, and we also have a satellite office in Sterling (please click here for our contact information).
NCAP serves hundreds of clients annually - an increasing number of whom are newly diagnosed, and many who are long-term survivors. As a small non-profit agency, we have the ability to offer our clients a high level of personal service and support from the most dedicated workers in the field. All case management services are provided at no cost to people living with HIV/AIDS. NCAP does not discriminate on the basis of age, gender, ethnicity, race, religion, sexual orientation, national origin, marital status, or disability.
What does NCAP provide for clients?
Though we are not a medical clinic, NCAP has many on-site services, as well as an extensive referral network. Our case managers can:
- Arrange transportation for appointments
- Assist with referrals to appropriate community housing resources
- Offer tools to take an active hand in your health and well-being
- Provide support in finding the doctor that's right for you
- Translate forms and work as an interpreter
NCAP is funded in part through the federal Ryan White CARE Act, which provides nationwide support to agencies that assist people living with HIV/AIDS. There are a number of federal and state programs that our case managers are familiar with, including:
- HOPWA (Housing Opportunities for People With AIDS)
- Ryan White CARE Act insurance
- ADAP (AIDS Drug Assistance Program)
- LEAP (Low-income Energy Assistance Program)
- PAPs (Prescription Assistance Programs)
- And many more.
NCAP takes the time to get to know the people we serve. Case managers meet with every client to assess their specific needs and a care plan is developed to help the client achieve their own personal goals and manage their health care. Mental health referrals, home visits, services for families, and emergency assistance are all available. When you sit down to speak with a case manager, your needs are the focus.
